Identification: The adaptor's model number PA-10 can be found on the Yamaha label on the
front of the adaptor. Only adaptors numbered with a date code between 2126 - 2319 are recalled. The date
code is inscribed on of the back of the adaptor.

Why the product is recalled: The casing of the adaptor can develop a crack which can expose the live electrical contacts inside.
Hazard: There is a risk of electric shock, serious injury or death if internal electrical contacts are exposed.
Action required: Consumers should stop using the adaptor immediately and disconnect the plug from the power outlet and the audio mixer.
