Leave of Absence Form

IMPORTANT INFORMATION WHEN APPLYING FOR LEAVE OF ABSENCE

Please note: the Leave of Absence Process is only applicable to Yamaha Piano Students and Yamaha Guitar Students (Private Courses).

A request for Leave of Absence during a student's Yamaha Music course will be approved or denied based on individual circumstances surrounding the type of leave requested. It is at the sole discretion of Yamaha Music School to approve or deny any leave request.

Students with approved leave will be provided with a credit to the equivalent value of the missed Private Lesson towards a future Semester. Customers are entitled to up to 2 credits per Semester.

• Maximum of 2 lessons per semester - any more than two lessons of absences across a semester and Yamaha will not be able to deliver the educational outcomes promised.
• 48 hours' notice must be provided - if a request form is provided without notice, the student will not be eligible to receive a credit.
